Why does tirzepatide cost different amounts?

Because the price belongs to the product, not the molecule. Zepbound is Lilly's tirzepatide for weight: FDA-approved for chronic weight management and for moderate-to-severe obstructive sleep apnea in adults with obesity[1] Lilly sells it to cash payers through LillyDirect at $299 to $449 per 28 days, depending on dose[2]. Mounjaro is the same ingredient for type 2 diabetes. FDA-approved for type 2 diabetes[5]. It is not approved for weight loss; Zepbound is the tirzepatide product for that. We found no LillyDirect self-pay vial for it, so its price is the list price or the savings-card price[6].

Compounded tirzepatide is a third thing again. Compounded drugs are made by state-licensed pharmacies or outsourcing facilities, are not FDA-approved and are not generic versions of Zepbound or Mounjaro. The 7 telehealth programs we track advertise it from $90 to $389 a month, usually with a membership or a long-term plan[8].

What is the cheapest way to get tirzepatide?

It depends on coverage. The lowest published price for each route, from the product pages:

  • Commercial plan that covers it: $25 for Zepbound with Lilly's savings card[3], and $25 for Mounjaro[6].
  • Medicare: $50 a month for the Zepbound KwikPen through the Medicare GLP-1 Bridge, for eligible Part D enrollees[4].
  • No coverage, brand-name: $299 a month for Zepbound 2.5 mg on LillyDirect, $449 at 7.5 mg and above with on-time refills[2].
  • Compounded: advertised from $90, membership extra; read what that means before comparing it with an FDA-approved product.

The full route-by-route list, including semaglutide and orforglipron, is on cheapest GLP-1.

Is there a generic tirzepatide?

No program we track sells an FDA-approved generic tirzepatide; every brand-name offer we track is Zepbound or Mounjaro from Lilly. Compounded tirzepatide is sometimes described online as "generic", but it is not a generic version of either brand and is not FDA-approved. The FDA says tirzepatide does not currently appear on its drug shortage list[11]. See is compounded tirzepatide legal?
