What about Rybelsus, Trulicity, Saxenda and Victoza?

These older brands have no manufacturer self-pay price, so they are not in the tables above. Trulicity lists at $1,006.93 a month, Saxenda at $1,349.02 and is being discontinued, and Rybelsus is being replaced by the Ozempic pill. See Rybelsus, Trulicity, Saxenda and Victoza cost, and the state of the GLP-1 market for what changes in 2027.

Why does the same drug have several cash prices?

A self-pay price is what a manufacturer or program charges when no insurance pays any part of the bill. For the brand-name GLP-1s it usually rises with the dose: Zepbound is $299 at 2.5 mg and $449 from 7.5 mg up on LillyDirect with on-time refills, and the Wegovy pill is $149 at 1.5 mg and $299 at 9 or 25 mg. Some prices are temporary or depend on refilling on time, so each drug page lists the conditions next to the number.

Is cash ever cheaper than insurance?

Sometimes. With commercial coverage, manufacturer savings cards can bring the copay to $25, but a high deductible can make the self-pay price lower for months. Our insurance vs cash guide shows the arithmetic, and the Medicare GLP-1 Bridge covers Medicare enrollees at $50.
