The GLP-1 market in October 2026: how prices fell, who sells direct, and what changes in 2027

Checked

The GLP-1 market in October 2026 is a cash market as much as an insurance one. Lilly and Novo now sell their weight-loss drugs directly from $149 to $449 a month, down from $399 to $549 for Zepbound vials in 2024. Medicare pays $50 a month for some through 2027, and list prices fall in 2027.

How far have GLP-1 cash prices fallen?

Before August 2024 there was no manufacturer cash price for a weight-loss GLP-1. Lilly launched Zepbound vials on LillyDirect on August 27, 2024 at $399 (2.5 mg) and $549 (5 mg)[1]. Novo followed with NovoCare Pharmacy on March 5, 2025, selling every Wegovy dose at $499[2].

Both then cut. Novo took Wegovy and Ozempic self-pay to $349 on November 17, 2025, with a $199 introductory price for new patients[3]. Lilly cut Zepbound to $299, $399 and $449 by dose on December 1, 2025[4]. Two pills arrived at $149 at their starting doses: Foundayo in April 2026[5] and the Wegovy pill[6]. The full dated record is in the price-change log and the price index.

Who sells GLP-1s directly to patients?

Three kinds of channel now publish prices:

  • Manufacturers: LillyDirect for Zepbound and Foundayo[7], and NovoCare Pharmacy for Wegovy and Ozempic[6].
  • The federal site: TrumpRx.gov lists the lowest manufacturer price for each drug and routes you to the maker[8].
  • Telehealth programs: they prescribe and either pass through the manufacturer price plus a membership, or sell compounded versions. Ro says its prices "match LillyDirect, NovoCare, and TrumpRx"[9].

Compare them on the programs hub or with the cost builder.

What happened to compounded GLP-1s?

Compounding grew during the shortages. FDA says tirzepatide and semaglutide no longer appear on its drug shortage list and has clarified its policies for compounders as supply stabilized[10]. Telehealth programs still advertise compounded versions, and we track their published prices on the compounded semaglutide and compounded tirzepatide pages. Compounded drugs are not FDA-approved; the legal position is explained in our compounded GLP-1 legal status guide.

What do Medicare and Medicaid cover now?

From July 1, 2026 to December 31, 2027, the Medicare GLP-1 Bridge gives eligible Part D enrollees Wegovy, Foundayo and the Zepbound KwikPen for a $50 monthly copayment, with prior authorization[11]. Medicaid coverage of GLP-1s for obesity is optional for states, and KFF counted 13 state programs covering them as of January 2026[12]; see the Medicaid guide. Medicare’s negotiated price for semaglutide, $274 for a 30-day supply, starts on January 1, 2027[13].

Which GLP-1 brands are going away, and which generics exist?

Novo is replacing Rybelsus with the Ozempic pill[14] and discontinuing Saxenda, with last wholesaler shipments in late January 2027[15]. Generic liraglutide exists for both Victoza (FDA approval December 23, 2024)[16] and Saxenda (Teva, August 28, 2025)[17]. There is no FDA-approved generic semaglutide or tirzepatide. Prices for the remaining older brands are on the Rybelsus, Trulicity and Saxenda page.

What changes on January 1, 2027?

Three dated changes are already announced:

  • Novo’s list price for Wegovy, Ozempic and Rybelsus drops to $675 a month, about 50% lower for Wegovy and 35% for Ozempic. Novo says self-pay prices do not change[18].
  • The Medicare negotiated price for the semaglutide group takes effect: $274 for a 30-day supply[13].
  • Two current offers end on December 31, 2026: Novo’s $199 introductory Wegovy price[6] and the current Trulicity Savings Card terms[19].

We will log each change in the price-change log on the day we see it on the source.
