Zepbound cost in October 2026: $299–$449 a month without insurance
As of October 19, 2026, a month of Zepbound bought without insurance from LillyDirect costs $299 at the 2.5 mg starting dose, $399 at 5 mg and $449 at 7.5 mg to 15 mg, if each refill is within 45 days. Vials and the KwikPen cost the same. With commercial coverage, it can be $25. Source: Eli Lilly and Company.

How much is Zepbound without insurance?
Lilly sells Zepbound directly to cash-paying patients through LillyDirect. The price depends only on the dose strength, not on whether you choose vials or the KwikPen: $299 for 2.5 mg, $399 for 5 mg and $449 for 7.5, 10, 12.5 or 15 mg[1]. Lilly says patients "can access all doses in either multi-dose KwikPen or single-dose vial at the same self-pay price"[4].
The $449 figure for the four higher doses is Lilly’s Zepbound Self Pay Journey Program price. It applies automatically on your first purchase and stays in place only if every refill is completed within 45 days of your previous delivery. If you refill late, LillyDirect charges the regular price: $499 for 7.5 mg and $699 for 10, 12.5 and 15 mg[1].
Lilly’s own pricing disclosure page states the same direct-to-patient prices without the Journey discount ($299, $399, $499, $699, $699 and $699 by dose) and gives a list price, or wholesale acquisition cost, of $499 to $1,086.37 per fill[2]. The list price is what Lilly charges wholesalers, not what a cash patient pays at LillyDirect.
What does a year of Zepbound cost?
Lilly defines a month as 28 days and four weekly doses, so twelve months of weekly treatment is 13 fills, not 12. At the on-time-refill price, 13 fills cost:
- $3,887 at 2.5 mg ($299 × 13)
- $5,187 at 5 mg ($399 × 13)
- $5,837 at 7.5 mg to 15 mg ($449 × 13)
These are arithmetic on published prices, not a prediction of your dose path. Your prescriber decides your dose and how fast it changes. Most people do not stay on one strength all year, so a real first year usually mixes several of these prices. Needles and syringes for vials, or pen needles for the KwikPen, are an extra purchase[1]. Use the comparison builder to total a specific mix.
How has the Zepbound cash price changed?
Zepbound has only had a direct self-pay channel since August 27, 2024, when Lilly launched 2.5 mg and 5 mg vials at $399 and $549 for a four-week supply[5]. On February 25, 2025, Lilly cut those to $349 and $499 and added 7.5 mg and 10 mg vials at $499 with on-time refills[6]. On December 1, 2025, it cut prices again to the current $299 / $399 / $449 structure[7], and on February 23, 2026 it added the KwikPen at the same prices[4].
Net effect for the starting dose: $399 to $299, a 25% drop in a little over two years. The late-refill penalty has also shrunk. In December 2025 Lilly listed regular prices of $599 to $1,049 for the higher doses[7]; on our October 19, 2026 check, LillyDirect showed $499 to $699[1]. We do not know the exact date of that change, so the chart only plots dated announcements and our own check.
How much is Zepbound with insurance?
If your commercial plan covers Zepbound, Lilly’s savings card can bring the cost down to as little as $25 for a one- or three-month supply of single-dose pens. Government beneficiaries are excluded, and savings are subject to monthly and annual limits[2]. LillyDirect shows the single-dose pen as an insurance product ("Insurance accepted") and the KwikPen as "Select insurance accepted | Self-pay accepted"[1].
If your plan does not cover Zepbound, the $25 card does not apply and the self-pay prices above are the relevant numbers. Check our guide to insurance versus cash before you decide.
Does Medicare pay for Zepbound?
From July 1, 2026 to December 31, 2027, the Medicare GLP-1 Bridge lets eligible Part D enrollees get select GLP-1s for weight reduction at a $50 monthly copayment. KFF reports that the drugs covered are all formulations of Foundayo and Wegovy and the KwikPen formulation of Zepbound, and that a prescriber must submit a prior authorization request[3]. Zepbound vials are not on that list. See the Medicare GLP-1 Bridge guide.
Is Zepbound cheaper through telehealth?
Not the medicine itself. Every telehealth program we track that sells Zepbound at the manufacturer price quotes the same $299 starting figure as LillyDirect, and Ro states its prices "match LillyDirect, NovoCare, and TrumpRx"[8]. What changes is the membership you pay on top. Hims and Hers, for example, charge $39 for the first month and then $149 a month, billed separately from medication[9]. A few programs list Zepbound far above the manufacturer price: Hims shows a Zepbound listing (with no device named) at $1,899 a month[9] and Fridays lists Zepbound at $1,828 a month on a month-to-month plan[10].
TrumpRx.gov lists Zepbound at $299 a month next to an "Original Price" of $1,087[11], which matches LillyDirect’s starting dose.
Is compounded tirzepatide the same as Zepbound?
No. Compounded drugs are made by state-licensed pharmacies or outsourcing facilities. They are not FDA-approved, are not generic versions of Wegovy, Ozempic, Zepbound or Mounjaro, and the FDA has not reviewed them for safety, effectiveness or quality. The FDA says tirzepatide does not currently appear on its drug shortage list[12]. Compounded tirzepatide prices are tracked on a separate page: compounded tirzepatide cost.

What should I do next?
- Check your coverage first. If a commercial plan covers Zepbound, a manufacturer savings card can cut the copay to as little as $25. See GLP-1 savings cards.
- On Medicare? Savings cards don’t apply; the Medicare GLP-1 Bridge sets a $50 copay for some weight-loss GLP-1s.
- Paying cash? Compare the self-pay routes in the table above, including membership fees, in the cost builder.
- Ask your prescriber which drug and dose fit you. GLP Observer compares prices only.

What is the Zepbound Self Pay Journey Program?
It is Lilly’s on-time-refill discount for 7.5, 10, 12.5 and 15 mg. You pay $449 a month as long as each refill is completed within 45 days of your previous delivery; otherwise the regular price applies.

Is there a Zepbound coupon or savings card?
Lilly’s own programs work as the coupon. With commercial insurance that covers Zepbound, the Zepbound savings card can bring it to as little as $25. Without coverage, LillyDirect self-pay is $299 to $449 a month. We do not list pharmacy discount-card prices, because they are not published with a date.
Does Medicare cover Zepbound in 2026?
